Patient Forms

In order to make your visit to our office as quick and convenient as possible, we have provided these forms for you to fill out prior to your appointment. After completing these forms, please print and bring them to your appointment.

Payment Options

We accept various PPO plans and are preferred providers of Aetna, Delta Dental, Cigna, and MetLife. As a courtesy, we will file your insurance for you and assist you in maximizing all benefits. We accept payment in the form of cash, check, most major credit cards, and offer affordable financing through CareCredit. If you have any questions regarding your payment options, please call our office at (972) 366-7743.

New Patient Information

New patients can expect their first visit to last about two hours.  During this time, you will receive a comprehensive exam and diagnosis and be presented with a customized treatment plan.  When you walk into our office, you will be greeted by our friendly front office team and walked through filling out any necessary paperwork.  You can speed up this process by completing the Patient Forms listed above prior to your appointment.

Upon early arrival, you may help yourself to our coffee bar, read a magazine, or watch a movie in the reception area.

Dr. Thompson will take the time to get to know you and take your dental history, and you will have the opportunity to address any of your dental concerns. Your appointment time is reserved for you exclusively. You will not feel rushed or confused about your visit or your dental health.

After you’ve met with Dr. Thompson, you will receive the appropriate and necessary diagnostic x-rays to assist the team during your visit.  Next, our hygienist will come in for a full oral inspection and evaluation, including some light probing of your teeth and gums.  Dr. Thompson will return for an oral cancer screening and comprehensive exam and will convey any areas of concern while using the x-rays and photos on the private computer screen as a visual aid.  Your treatment options will be discussed with you using patient education tools such as models and examples, and together you will develop a treatment plan to achieve optimal oral health.

Following your exam, our expert financial coordinator will discuss your treatment plan and present your payment options to transition smoothly to your next dental appointment should you have treatment needs.
